Hope House Home Healthcare provides Registered Nurse Assessments for each client to determine their individual needs and compose a comprehensive, individualized Plan of Care. Our Registered Nurses also make regular supervisory visits to evaluate the quality of care being delivered and the client’s level of satisfaction. We offer evening and weekend care services upon request and our Registered Nurses are available for consultation during normal hours of operation. We provide care in-home and in residential and non-residential care facilities.

Personal/Attendant Care

Provides Assistance with Daily Living

Our Personal Care Attendants assist the elderly, convalescents, and persons with disabilities with daily living activities in-home and in care facilities. They also provide assistance at nonresidential care facilities and may advise families, the elderly, convalescents, and persons with disabilities on nutrition, hygiene, and household activities. We help our clients with:

  • Bathing, grooming and dressing
  • Mobility assistance
  • Assistance with position changes and getting in and out of bed
  • Incontinence care, toileting needs, and safety in the bathroom
  • Assistance with preparing and serving meals, and support for special diets
  • Laundry care
  • Medication reminders and management
  • Assisting with outings, social events, shopping trips, and other activities of interest to maintain engagement both physically and emotionally
  • Support for physician-prescribed nutrition and exercise programs
  • Change in condition monitoring
  • Fall risk assessment and assistance with fall prevention
